INFORMATION FOR INCOMING ERASMUS STUDENTS FROM FACULTY OF PHARMACY AND FACULTY OF HEALTH SCIENCE (nursing, electroradiology):

REQUIREMENTS:
- Application form (401 KB)
- Copy of passport or ID
- A confirmation of Third Party Insurance (if you don’t have it – there will be opportunity to buy it in MUG) and an Emergency Insurance certificates (European Health Insurance Card (EHIC) or a Provisional Replacement Certificate of the EHIC. Primary healthcare (PHC)
- A confirmation of English Language knowledge (at least B2 level) – we accept confirmation issued by university language department
PHARMACY:
Clear, up-to-date results of tests for ALT, AST, GGTP, creatinine, general urine tests, potassium, calcium, CK, HIV, HCV, HBC TOTAL, and Hepatitis B – conducted no earlier than 14 days before arrival and no older than 30 days prior to the start of studies. The results must have a clear, easy-to-read copy, including the date of the test and the owner’s full name.
NURSING:

ELECTRORADIOLOGY:
Clear, up-to-date results of blood tests (MORPHOLOGY, RETICULOCYTES, and Hepatitis B), an ophthalmologic examination for ionizing radiation, and an ECG – conducted no earlier than 14 days before arrival and no older than 30 days prior to the start of studies. The results must have a clear, easy-to-read copy, including the date of the test and the owner’s full name.

2. All health tests must be up-to-date and delivered before arrival to MUG to the e-mail: HEALTH SCIENCES erasmus.wnoz@gumed.edu.pl. PHARMACY: erasmus.wf@gumed.edu.pl
3. Student must arrive to the Medical University of Gdańsk one week before the start of academic year. MUG organises the Orientation Week when there will be scheduled a physician’s visit (occupational medicine)
4. Students must take the original of Health Tests to a visit in order to be confirmed/ approved by a occupational medicine physician
If an occupational medicine physician during the visit states that the examinations are incomplete or incompatible with our standards – a student will be obliged to perform supplementary examinations at his own expense.
